Enrollment is an easy two step process:
STEP 1: Midwest Capital Enrollment
Secure online enrollment for your Midwest Capital 403(b) is easy and takes about 10-15 minutes. If your online enrollment process is interrupted for any reason, you may log in and resume where you left off. If you prefer to enroll using a paper enrollment form, a copy is available through your payroll department or by contacting Midwest Capital at 1 (877) 269-4032.

STEP 2: Salary Reduction Authorization
Obtain your district Salary Reduction Authorization form from your payroll department or TSA Consulting Group's website (1. Chose your ISD 2. Chose your district 3. Select "List of Forms" 4. Select "Salary Reduction Agreement"). In the upper right hand corner write Midwest Capital Advisors. Complete it and submit it to payroll. If your payroll department requires a signature from Midwest Capital, the form may be faxed to Midwest Capital at (616) 454-9608 and we will countersign and fax the signature copy back to either you or your district at your direction.
